Our Passion
The mission at San Gabriel Rehabilitation and Care Center is to enhance the quality of life for each resident and promote feelings of productivity and life satisfaction. We offer a combination of long term care, therapy, purposeful groups, and one-to-one interactions aimed at maintaining both physical and cognitive ability.
In our Rehabilitation Program, the clinical team will evaluate each resident’s unique circumstance to develop and meet individualized goals. During treatments, therapists will work toward improving independence, strength and confidence.
Long-Term Care assists those who may benefit from 24- hour nursing availability and assistance with meals, medications or other activities of daily living. The environment at San Gabriel is designed so that residents may live comfortably, as a part of a community.

Planned admissions are accepted 24 hours a day, 7 days a week, including holidays.
Visiting Hours
Based on the daily activities and needs for our residents, recommended visiting hours are 8:00 a.m. until 8:00 p.m.

Mission Statement
San Gabriel Rehabilitation and Care Center is proud of its history in the community and we treasure our relationships with residents and families. We strive to maximize and enhance the quality of life of our residents in a compassionate and professional environment. The need for long-term care, skilled nursing, or rehabilitation may not occur under ideal circumstances. A fall, stroke, or brief illness can deprive us of our customary feeling of independence and security. Fortunately, we have a team of trained professionals that will assist in developing individualized plans of care, in order to achieve desired goals.
We strive to maintain the highest standard of care to help our residents get back into the community or to create a healthy long term living environment.
