San Gabriel Rehabilitation and Care Center offers individualized care to include daily living assistance, medication management and specialized levels of care.
We strive to provide each patient with the tools needed to achieve personal independence to the fullest extent possible.
Rehabilitation
We have a state of the art therapy gym with professional, caring therapists working with our residents 5 to 6 days a week. San Gabriel is very proud of the positive outcomes our patients achieve because of the therapy they receive here.
Wound Care
San Gabriel is known as a premiere facility for healing wounds acquired in the community. We have a skilled, multi-faceted team that knows how to get problem wounds healed.

Long-term Care
Not only is San Gabriel an excellent rehabilitation center it is also has two dedicated long-term care halls. These halls are for residents who need 24 hour care. We have dedicated staff that just focuses on these residents to make San Gabriel truly their home.
Short-term Care
Individualized therapy and treatment is provided by skilled clinicians in our motivational therapy gym. Our facility is designed with the very latest therapy assessment, monitoring, and treatment modalities. Our rehabilitation strategy is tailored to the individual. We do this by simulating an environment that resembles the home environment as much as possible.
Physician Services
San Gabriel has partnered with local hospital systems to ensure continuity of care. These independent physicians also utilize nurse practitioners so when they not here, they have licensed and certified advanced practice nurses at the facility to ensure that trained eyes are available and ready to assist in making informed healthcare decisions.
